Our Referrals Department is here to support you through the referral process for any specialty care you may need. We understand that navigating referrals and insurance requirements can feel overwhelming.

Our team is here to ensure a smooth transition from primary care to specialty care, including assistance with insurance authorizations and scheduling support.

What is a Referral?

A referral is an official recommendation from your primary care provider (PCP) for you to see a specialist or complete specific tests, imaging, or consultations. 
Many insurance plans require a referral before covering specialist visits or certain procedures. 

Our team helps by

managing paperwork

coordinating with insurance

obtaining authorizations when needed

sending documentation to the specialist office

How the Referral Process Works

Initial Consultation with Your Provider

During your appointment, you and your MCHC provider will discuss your health needs. 
Your provider will explain why a specialist or test may be recommended and answer any questions. 

Referral Processing

After the referral is ordered, our Referral Coordinators will: 
submit the referral request  
send supporting documentation  
request insurance authorization if required  

  • submit the referral request

Confirmation & Scheduling

Once insurance approval is received (if required), our team will send all necessary documents to the specialty facility. 
This includes: 
provider referral order  
medical documentation  
insurance authorization  
If the specialty office allows scheduling by our team, we will attempt to book the earliest available appointment. 
Appointment details should be communicated by: 
MyChart  
phone  
mailed letter  
If the specialist requires the patient to book directly, include clear contact information for the patient. 

Follow-up Care

After the specialist visit, your MCHC provider will follow up with you to review findings and treatment recommendations. 
This ensures your care remains coordinated and fully supported.
