Patients should arrive 15 minutes early

Items to bring to your first visit:

Photo identification

Health Insurance Card (if applicable)

Social Security Card (if applicable)

Medical or dental history

All current medications

Adult guardians for minors

Immunization records

Need help with Insurance?

Our team can help patients apply for state-sponsored health insurance plans if needed.
