Treat staff, patients, and visitors with courtesy and respect.
Use respectful language and behavior at all times.
Arrive on time for appointments and notify us if you need to cancel or reschedule.
Provide accurate and complete health, insurance, and contact information.
Respect the privacy and confidentiality of other patients.
Refrain from disruptive, threatening, abusive, discriminatory, or violent behavior.
Do not bring weapons, illegal drugs, alcohol, or tobacco products into the facility unless otherwise permitted.
Failure to follow this Code of Conduct
may result in corrective action, including restricted access to services when appropriate and consistent with applicable laws and regulations.
